{"created":"2023-06-20T15:46:22.084343+00:00","id":971,"links":{},"metadata":{"_buckets":{"deposit":"ed9530c2-0163-4891-a02a-2fa4c3c013c0"},"_deposit":{"created_by":4,"id":"971","owners":[4],"pid":{"revision_id":0,"type":"depid","value":"971"},"status":"published"},"_oai":{"id":"oai:uoeh-u.repo.nii.ac.jp:00000971","sets":["13:14:53"]},"author_link":["4788","4791","4794","4789","4792","4787","4793","4786","4795","4790"],"item_10001_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2022-09-01","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"3","bibliographicPageEnd":"299","bibliographicPageStart":"293","bibliographicVolumeNumber":"44","bibliographic_titles":[{"bibliographic_title":"産業医科大学雑誌"},{"bibliographic_title":"Journal of UOEH","bibliographic_titleLang":"en"}]}]},"item_10001_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"84歳，女性．初診数ヶ月前より全身紅斑が出没していた．初診時，38℃台発熱，意識レベル低下，全身に一部紫斑を混じる粟粒大紅斑が播種していた．WBC（8,200/μl），異型リンパ球（3%），sIL-2R（4,030U/ml），CTで全身リンパ節腫大があった．左鼠径リンパ節生検では，リンパ節の構造が破壊され，高内皮細静脈の増生と異型リンパ球の浸潤がみられ，Angioimmunoblastic T-cell lymphoma （AITL）と診断した．AITLは発熱，全身性リンパ節腫脹などの激しい全身症状を呈する比較的稀な末梢性Ｔ細胞リンパ腫である．節外病変として，約半数で皮疹を呈し，初発症状の一つとなるが，多彩な臨床像をとり，非特異疹も多く診断に苦慮する場合がある．皮膚病理組織学的所見でAITLの特異所見が得られない場合，リンパ節の特徴的な組織像により診断される．皮疹の遷延，全身症状，異型リンパ球出現など血液検査値の異常が続く場合には，リンパ腫も鑑別として挙げる必要がある．\nA 84-year-old female noticed erythema over her whole body for several months and was referred to our department for evaluation of her skin eruption. A physical examination revealed millet-sized erythematous papules and macules all over her body, a high body temperature, and a decreased level of consciousness. A laboratory examination showed an elevated white blood cell count (8200/μl), atypical lymphocytes (3%) and sIL-2R (4030U / ml). Computed Tomography showed systemic lymphadenopathy. A lymph node biopsy taken from the left inguinal lymph node revealed destruction of the lymph nodes, enlargement of the high endothelial venules, and atypical lymphocyte infiltration. Based on the clinical findings and laboratory examination, we diagnosed angioimmunoblastic Tcell lymphoma (AITL). AITL is a relatively rare peripheral T-cell lymphoma with severe systemic symptoms such as fever and lymph node swelling. While approximately half of all cases experience skin symptoms, which are one of the initial symptoms, it is difficult to determine the diagnosis due to the various clinical features or many non-specific rashes. We should keep in mind a differential diagnosis of lymphoma in cases of the presence of persistent eruption, systemic symptoms, and the existence of atypical lymphocytes in peripheral
