{"created":"2023-06-20T15:46:08.652208+00:00","id":733,"links":{},"metadata":{"_buckets":{"deposit":"5b253ab3-f69a-42ea-88da-831065683d77"},"_deposit":{"created_by":4,"id":"733","owners":[4],"pid":{"revision_id":0,"type":"depid","value":"733"},"status":"published"},"_oai":{"id":"oai:uoeh-u.repo.nii.ac.jp:00000733","sets":["13:24:33"]},"author_link":["2336","2334","2337","2338","2335","2339"],"item_10001_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2016-03-01","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"1","bibliographicPageEnd":"34","bibliographicPageStart":"25","bibliographicVolumeNumber":"38","bibliographic_titles":[{"bibliographic_title":"産業医科大学雑誌"},{"bibliographic_title":"Journal of UOEH","bibliographic_titleLang":"en"}]}]},"item_10001_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"組織線維化は脳を除くほぼ全身の主要な臓器に生じ,線維化を起こした臓器は最終的に機能不全に陥る.腎線維化は急性腎障害の瘢痕形成や慢性腎臓病の進行に関与しており,腎線維化が進むことは腎機能が低下することを意味している.一度生じた線維化は不可逆的で,腎臓に関しては透析導入や腎移植を迫られてしまう.したがって,腎線維化の機序を解明することは非常に重要な研究課題である.線維化の機序として線維芽(筋線維芽)細胞の活性化と増殖や,過剰な細胞外基質産生が線維化を招くことは多くの臓器で共通したことであるが,そこに至るまでの経路には腎臓特有の原因も存在すると考えられている.つまり,腎臓には尿細管細胞,メサンギウム細胞,エリスロポエチン産生細胞といった他臓器には無い細胞が存在し,それらが線維化に関与していることが分かりつつある.腎臓特有の状況や細胞内外の情報伝達系を中心とした線維化には,transforming growth factor-β (TGF-β) によって誘導された上皮間葉移行や,wingless/int (WNT) シグナル,さらには,慢性腎臓病に伴う炎症,腎性貧血や尿毒症による機序が存在しており,それらについて論述する.最後に,腎線維化に関与している分子を標的とした治療法として,抗TGF-β抗体やセリン/スレオニンキナーゼ,mammalian target of rapamycin (mTOR) について述べる. \nFibrosis occurs in systemic tissues other than the brain and finally induces dysfunction of the fibrotic organ. Kidney fibrosis is related to scarring after acute kidney injury and the progression of chronic kidney disease. Kidney function decreases with the progression of kidney fibrosis. As fibrotic tissue cannot return to its original status, advanced kidney fibrosis requires the administration of dialysis or kidney transplantation. Thus, elucidation the mechanism of kidney fibrosis is an important research theme. The proliferation and activation of (myo) fibroblasts and the excessive production of an extracellular matrix are common mechanisms in fibrosis in many organs, but it seems that kidney fibrosis has specific pathways. Tubular epithelial, mesangial cells, and erythropoietin producing cells, which exist only in the kidney, participate in forming kidney fibrosis. This review highlights an understanding of the cells and their underlying mechanisms, which are specific to kidney fibrosis process: transforming growth factor-β (TGF-β), epithelial-mesenchymal transition, wingless/int-1 (WNT) signaling, renal anemia, and uremia. Finally, we describe potential therapies that focus on the mechanisms of kidney fibrosis: anti-TGF-β antibody and mammalian target of rapamycin (mTOR).
